OFFICE POLICIES

We are pleased that you have chosen Our Children’s Clinic to provide pediatric care for your child. Our Staff is committed to making your visit as pleasant as possible. We strive to provide the best medical care for our patients. While making every effort to provide service promptly, occasionally emergencies in the office and in the hospital do occur and we must adjust our schedule. We appreciate your understanding and patience. If you have any suggestions or feedback for our office, please let us know.

Office Hours
Our office is open 8 AM to 5 PM Monday through Friday. Patients are scheduled from 8 AM through 12:00 PM and 1:30 PM through 4 PM. Our phones remain open for messages and appointment scheduling during lunch hour. We are closed in observance of major holidays.

Cell Phone Use
Cell phone use is strictly prohibited beyond the waiting room areas in accordance with HIPPA regulations.

New Patients
First time patients are asked to arrive at least 30 minutes prior to their appointment time to allow adequate time for completing the initial paperwork. In order to maintain continuity of care, we ask that you bring a list of current medications, any relevant records including any test results, a current insurance card, and a photo ID.

Minors in office
For safety reasons, we depend on parents to properly supervise their child(ren) at all times in our office. Under no circumstances may a child under the age of ten be left unattended. Minors 15 years of age or younger must be accompanied by a parent or legal guardian. We require a consent form signed by a parent or legal guardian to provide medical care to minors 16 and 17 years of age when the parent cannot be present for a sick visit. Parents or legal guardians need to be present during all Well Child Visits including those for our adolescents.

Changes in Address, Billing information, or Contact Information
Please notify our office in writing of any changes of address, telephone numbers, billing or contact information. It is important that this information is correct should we need to contact you regarding your child's health.

Insurance Policies and Forms
Our office will submit claims for insurance carriers with which we participate. At Our Children’s Clinic, we are dedicated to offering quality health care. Often this care may be beyond the medical standard of care. It is your responsibility to know your insurance plan benefits. Comprehensive physicals, some immunizations, certain procedures, and particular laboratory tests may not be covered. Prior authorizations may be completed at the discretion of the providers. Services or therapies that we provide or recommend may or may not be covered by your insurance policy. We also require proof of current insurance at check-in for every visit. It is essential that you provide all the necessary information about your insurance, both primary and secondary. Since changes in insurance coverage are frequent, it is our policy to obtain a copy of your card(s) at each visit. In case of a new policy, a copy of the enrollment form specifying insurance company, their name and phone number, employer and his/her phone number, insured employee name, date of birth, and social security number will be required.

Medical Records and Forms
All medical record requests must be on a HIPPA approved form which must be completed and signed by the parent or legal guardian. Please allow at least five business days for processing.

Our providers will complete physical forms to participate in sports and school related activities provided they are current on their well child visits. Please allow two-five days for processing. If a specific form is required, please bring it to the office. NC athletic participation forms and kindergarten forms are available for download on the Forms section of our website.

Medical records released to a new provider, a specialist, or school will be forwarded at no charge as a courtesy to our patients. Medical records released to the patient, some insurance companies, law firms, or other miscellaneous requests are subject to copying fees. FMLA Forms, 504 Letters, and letters requested by insurance companies can be completed for you but there will be a $20 fee.

After Hours Care
After hours and during holidays call 252-355-3773 and you will be able to speak with Vidant Health Nurse Triage.  You may choose to be triaged or you may also request to speak directly to the provider on call. For an acute illness, appointments can be made for the after hours pediatric clinic by the nurse triage personnel. We are proud to be able to provide our patients with direct provider access and evaluations for acute problems after regular hours. This service is not to be used for medication refills or appointment scheduling during regular office hours.

IN A LIFE THREATENING SITUATION, PLEASE CALL 911 IMMEDIATELY.
